Effective November 5, 2021, CMS requires all Medicare and Medicaid certified providers and suppliers to have a policy on COVID-19 vaccination. All facility employees, licensed practitioners, students, trainees, and volunteers will be required to provide proof of vaccination or a religious/medical exemption by January 4, 2022. In addition to staff, proof of vaccination or exemption will be required for all individuals who provide care, treatment, or other services for the facility and/or its patients under contract or other arrangements.
